6-Month Auto-Renew Membership
This package allows you to take 10 Pole Dance Academy, Specialty Pole, Open Pole Play, Dance and Fitness classes every month.
Contract Agreement:
• All Membership Contracts are a Six (6) Month commitment with an automatic renewal. The is Non-Negotiable.
• An Enrollment Fee of $30.00 + 1st Month is due at signing.
• A valid credit card must be kept on file and must agree to monthly automatic payments for membership. If a card is declined, you will be charged a $35.00 Fee.
• Membership Cancellation requires a 30-Day Written Notice and is subject to a $150.00 Cancellation Fee.
• If Cash Payments is required for memberships; entire balance must be paid in full.
• Memberships cannot be suspended or put on hold.
• Monthly classes do not roll over, any unused classes will be lost.
• Memberships cannot be cancelled within the first 4 Months FOR ANY REASON except medical. Doctor’s note required.
• Memberships do not include Workshops or Special Events.
• Additional classes can be purchased at a discounted rate of $15.00/class.
• 15% Merchandise Discounts on all Taboo Logo Apparel
• Free Vacation Voucher available with 6 Month commitment and will be dispersed after 6 month completed or
